Privacy Policy
This is an agreement between Omega Aesthetics & Wellness and the patient in the agreement, the words “you”, “your”, and “yours” meant he debtor/patient. The word “account” means the account that has been established in your name and to which charges are made and payments collected. The words “we”, “Us”, and “Our” refer to Omega Aesthetics & Wellness.
By executing this agreement, you are agreeing to pay for all services that are received. Medical consent for treatment. I am voluntarily seeking medical and cosmetic treatment and hereby consent to medical treatment, minor procedures and other healthcare services. I have to right to refuse specific treatments and procedures. This Agreement can be revoked by me at any time with the written notification and valid until revoked.
Payment Methods Accepted:
We accept cash, Visa, Mastercard, Discover, American Express, and Care Credit, Apply Pay and. Venmo
Past Due Accounts:
If your account becomes past due we will take any and all necessary steps to collect this debt. If we refer your account to an outside collection agency, all future correspondence regarding the debt will need to be made directly through the collection agency. We reserve the right to cancel your privileges to make charges against your account at the time due to delinquent balances. If a balance remains unpaid, future care you may need by our office could be affected.
Waiver of Confidentiality :
You understand if this account is submitted to a collection agency and/or attorney if there is litigation in court, or if your past due status is reported to a credit reporting agency, the fact that you received treatment at our office may become a matter of public record.
Permission to contact via cellular phone/email:
Since most people only have a cell phones, we will need your permission to call your cell phone. Signing this agreement will allow us to call you regarding medical or billing information. Please let us know if you have any questions or would like to discuss this.
By providing your mobile phone number and email address, you are agreeing to receive automated emails and text (SMS) messages to your mobile phone and other wireless devices, for the purpose of confirming appointments and to discuss billing issues.
Providing an email address or phone number is necessary to receive appointment service notifications.
You may opt out of receiving text (SMS) messages at any time by relying with the word STOP from the mobile device receiving the messages. However, you acknowledge that opting-out of receiving text (SMS) messages may impact your experience with the services(s) that rely on communication via text (SMS) messaging. I can withdraw my consent for receiving text (SMS) messages at ay time.
Social Media:
I hereby authorize Omega Aesthetics Utah LLC to use my photo, videos, and/or information related to my experience with Omega Aesthetics & Wellness services and staff. I understand this information may be used in social media including but not limited to instagram, facebook, and Tik Tok
My consent is freely given as a public service to Omega Aesthetics & Wellness without expecting payment. I release Omega Aesthetics & Wellness and their respective employees, officers and agents from any and al liability which may arise.
I understand that I can revoke this release any time in writing and that the use of any of my photos or other information authorized by this release will immediately cease.
If I do not agree to this I will simply let a member of the staff know so they can make an alert of that in my customer profile and medical chart.
Return Policy:
All services, packages and product sales are final. Services/Packages- we do not offer refunds on services rendered or packages rendered, even if you are unhappy with the outcome. We may offer credit towards another service if appropriate.
Products may be returned for in store credit within 14 days of purchase in accordance with federal law. we do not offer refunds on any prescription products for any reason.
Cancellation/ No Show Policy:
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment 24 hours in advance, you may be subject to a $50 fee. Omega Aesthetics & Wellness requires that a credit card number be kept on file to reserve your scheduled service.
If an appointment is not cancelled at least 24 hours in advance, you will be charged a $50 fee.
Scheduled Appointments:
We understand that delays can happen. However, we must try to keep the other patients and medical staff members on time. If a client is more than 10 minutes late, we have the right to reschedule the appointment.
If you are more than 15 minutes late for an appointment, Omega Aesthetics & Wellness will charge a $50 fee.
Omega Tier Memberships:
If you cancel your membership the day you receive treatment, you will not be eligible for the discount within that tier. Membership banked money is non-refundable but can be used towards any services or product. If you would like to cancel your membership, we ask that you give enough notice to cancel before your next billing date.
Effective Date:
Once you have signed this agreement, you agree to all the terms and conditions contained herein and the agreement will be in full force and effect.
Your signature indicates that you have read and understand the financial policy, medical consent for treatment and assignment of benefit of Omega Aesthetics & Wellness. This agreement can be revoked by you at any time with written notification and is valid until revoked.
